I'm so sorry that Dorothy is in pain. My sister was given pain meds she could give sub-q as an injection when one of her Dobes needed pallative care. I would ask the vet if something you could inject at home might be a possibility. I also know that they will sometimes give Tylenol III to dogs...not sure how well that works.
